Assumptions and limitations

  • This six-item 0–2 questionnaire is a daily observation aid used by the cited veterinary practice. The total runs from 0 through 12, with discussion bands at 0–5, 6–8 and 9–12.
  • The captured selections allow whole ratings only. Other practice questionnaires, including HHHHHMM and Lap of Love, use different scoring systems; their thresholds cannot be substituted here.
  • Observe each item separately. Problems breathing, pain, inability to drink or elimination changes require veterinary attention regardless of the total.

Common questions

Can this score decide treatment?

No. Discuss the individual observations and changes over time with your veterinarian. A total cannot confirm comfort or decide end-of-life care.
