SERVICE AGREEMENT
North Star Wellness and [Client name]

1. PARTIES
This agreement is between North Star Wellness (the "Provider") and [Client name] (the "Client").

2. SERVICES
The Provider will provide the following services: Six 60-minute private wellness coaching sessions, one per week, in person or by video.
Services begin on 2026-09-01.

3. FEES AND PAYMENT
The total fee is $720.00 CAD.
A deposit of $360.00 CAD (50%) is due when the booking is confirmed. The remaining $360.00 CAD is due before the start date.
Fees do not include taxes unless stated. Any taxes required by law will be added.

4. CANCELLATIONS
The Client may cancel or move a session with at least 24 hours' notice at no charge.
Cancellations with less than 24 hours' notice, and missed sessions, are charged 50% of the session price.
If the Provider cancels, the Client may choose a new time or a refund for that session.

5. RESCHEDULING
Either party may ask to move a session with at least 24 hours' notice.
The Provider will offer the next available time that works for both parties. Moved sessions keep the same fee.

6. CONFIDENTIALITY
Both parties will keep private any personal, business, or health information shared during the services.
Information may be shared only with the other party's written permission, or when the law requires it.

7. LIMIT OF RESPONSIBILITY
The Provider will deliver the services with reasonable care and skill.
To the extent the law allows, the Provider's total responsibility under this agreement is limited to the fees paid by the Client, which is $720.00 CAD.
Nothing in this section limits responsibility that cannot be limited by law.

8. ENDING THIS AGREEMENT
Either party may end this agreement with 7 days' written notice. The Client pays for services delivered up to the end date, and any unused prepaid fees are refunded.

9. GOVERNING LAW
This agreement is governed by the laws of Ontario, Canada.
If any part of this agreement cannot be enforced, the rest still applies.
